Physical Exam & Immunization Requirements
To enroll in District 111, students must provide a current physical examination and immunization record before starting school. This applies to students enrolling for the first time, transferring into the district, or changing schools within the district due to a move.
Physical Exam and Immunization Requirements
- Illinois School Code: Requires submission of a physical exam report and immunization dates for school attendance. Immunization requirements may change; for the latest information, contact the Illinois Department of Public Health.

Grade-Specific Requirements

Preschool/Early Childhood:
- Physical exam dated 8/18/25 or later
- Diabetes and lead screening required
- Recommended tuberculosis (Mantoux) test
- Immunizations: DPT/DTaP (4 doses), Polio (3 doses), Varicella (1 dose), MMR (1 dose), Hepatitis B (3 doses), Hib (1 dose), PCV (1 dose)

Kindergarten (or first entry in Illinois school):
- Physical exam dated 8/18/25 or later
- Diabetes and lead screening required
- Recommended tuberculosis (Mantoux) test
- Vision exam by an optometrist, ophthalmologist, or physician
- Immunizations: DPT/DTaP (4 doses), Polio (4 doses), Varicella (2 doses), MMR (2 doses)
- Dental Exam Required

- Dental Exam Required

- Physical exam dated 8/18/25 or later
- Diabetes screening required
- Recommended tuberculosis (Mantoux) test
- Immunizations: Tdap (1 dose), Polio (3 doses), Varicella (2 doses), MMR (2 doses), Hepatitis B (3 doses), MCV (1 dose after age 10)
- Dental Exam Required
Important Dates
- Start Date: Students must meet these health requirements by 10/15/2026.
- Parent/Guardian Action: Ensure the health history portion of the physical form is completed and signed.
A dental examination, conducted by a licensed dentist, is required for all children in kindergarten, second, and sixth grade and must be completed by May 15 of the school year.
