Student Accident Medical Insurance Program
The Student Accident Medical Insurance Program offers optional coverage for students in the event of an injury during school activities or daily routines.
Accidents can happen during recess, field trips, sports, or regular school activities. This program helps provide coverage so students can receive care when needed.

Any enrolled student is eligible for coverage
Enrollment Information
Enrollment is completed by the parent or guardian, who is also responsible for payment of premiums.
To enroll:
- Visit: www.k12specialmarkets.com
- Click Coverage Details
- Select your state and choose your school/district
- Review available plan options
- Complete the application online or print and mail it
- Submit payment and print your ID card
Important Information
- This insurance is secondary to any other health insurance you may have
- Benefits are limited and may not cover 100% of expenses
- Claims must be reviewed and approved based on policy provisions
Filing a Claim
If your student is injured and you need to file a claim:
- Complete the claim form in full (all sections required)
- Submit medical bills (HCFA-1500 or UB-04 forms)
- If you have other insurance, submit claims there first
- Include Explanation of Benefits (EOB) from your primary insurance
- Submit all documents within 90 days of the first treatment
Incomplete forms or missing documents may delay processing.
